 Mornings:  Mon-Sat    8:00am-1:00pm

 Evenings:   Mon-Sat    3:00pm-7:30pm

Referrals
You DO NOT need a referral from your doctor before coming to our office. You may receive Chiropractic and physical therapy care without your physician’s referral.

Payment
For the part of your treatment that your insurance does not cover, or for any of our products and supplies, we accept payment via cash and credit card. Payment is expected at the time of service.

Your first visit:
Please plan to arrive at our clinic 15 minutes prior to the start of your first appointment to complete your new patient paperwork, which includes information about your current chief complaint, past medical history, family medical history, current functional status and pain level. You may also download the new patient paperwork here and bring them completed and ready to submit on your first visit.

 On your first visit please bring:

• Any imaging (X-Rays, CT Scans, MRI’S) taken for current chief complaint
• Health insurance Card
• Health history forms (if filled out from our website)
• Wear comfortable clothes
